Tele-Health Vitals Monitoring
Tele-Health Vitals monitoring enables doctors, health care providers and caregivers to monitor the daily vitals signs of their clients with a wide range of chronic illnesses including; diabetes, congestive heart failure, blood pressure, and asthma.
How Tele-Health Vitals Monitoring Works
- Home Buddy installs a convenient, easy to use modem in the client’s home. The modem plugs into a telephone jack.
- The client takes their vitals and, with the touch of a single button on the modem, downloads the readings to the Home Buddy Care Center.
- Home Buddy's’s team of clinicians and Care Center representatives watch the vitals and determine if they are in line with expectations. If our system algorithms determine they are out of line, we contact the client to ensure they are OK and then contact clinical professionals to triage the client helping them avoid hospitalization. Home Buddy's’s active monitoring ensures that the client gets the help that they need.
- If we do not receive vital signs within a specific time frame and according to their Care Plan, we call the client and give them a friendly reminder to take their vitals. We then follow up as necessary with caregivers or health care professionals.
Tele-Health Products
Blood
Pressure / Heart Rate Monitoring – The customer will put the cuff on
their left arm with the red strip facing them. They will press start and the
machine will take the reading and send the signal to the care center. The
machine is wireless, so the customer can take the reading form anywhere in the
home. Family members, nurses and the customer can view all of their readings
on-line also.
Glucose Monitoring – With Glucose Monitoring the customers can use their own
glucometer (in most cases). They can take their reading anywhere, anytime. The
customer plugs their glucometer into our unit. It will send the readings they
have taken to the care center. Twenty percent of patients over age 65 have diabetes, Thirty two
percent of Medicare dollars are spent on Diabetes patients.
Congestive Heart Failure / Weight Monitoring – We can
monitor the customers’ weight to provide
rapid and early detection of any complications before they become severe and
require hospitalization. The average cost is 8-10K per hospitalization for
Congestive Heart Failure. Forty three percent of Medicare dollars are spent on
Chronic Heart Failure patients.
Care
Pod- The
care pod is connected to the customer’s home phone line. When they take a
reading it is then sent to the care center through the care pod. The care
center will check the readings and let the selected responders (nurses, aids,
family members…) know if there is an abnormal reading.
